Position Overview

We are seeking a compassionate, skilled, and reliable Licensed Practical Nurse (LPN) to join our hospice team in a Part-Time/PRN capacity. This field-based role focuses on providing skilled nursing visits to patients in their homes. The LPN will follow individualized care plans, deliver direct nursing care, monitor patient conditions, and communicate effectively with the supervising RN. This is an essential role in ensuring the comfort and well-being of our hospice patients as part of a collaborative care team.


Compensation

  • Hourly Rate: $27.00 – $31.00/hour 
     


Work Schedule

  • Flexible scheduling – visits may occur any day of the week
     
  • Visit hours typically fall between 7:00 AM – 7:00 PM, depending on patient needs
     
  • Part-time hours vary based on caseload and availability
     


Key Responsibilities

  • Provide skilled nursing care in accordance with established care plans
     
  • Conduct scheduled home visits to hospice patients, delivering routine and chronic care
     
  • Perform wound care and other nursing procedures as directed
     
  • Accurately document observations, interventions, and patient responses in EMR
     
  • Monitor patients for any changes in condition and promptly report to the supervising RN
     
  • Follow all safety, infection control, and clinical best practices
     
  • Maintain communication with interdisciplinary team to ensure quality patient outcomes
     
  • Retrieve and manage necessary supplies before heading into the field
     
  • Educate patients and caregivers on basic health management and comfort techniques
     
  • Uphold patient dignity and emotional well-being during all interaction

Qualifications

  • Current LPN license in the state of Georgia (required) or Compact
     
  • Experience in hospice or home health preferred
     
  • Ability to identify and escalate changes in patient condition appropriately
     
  • Skilled in following detailed care plans and collaborating with supervising RNs
     
  • Excellent documentation, time management, and communication skills
     
  • Reliable transportation and valid driver's license (field-based role)
     
  • Compassionate, professional, and committed to quality end-of-life care
