Job Summary:
Primecare Home Care is currently seeking a dedicated and detail-oriented individual to join our team as a GAPP Care Coordinator. In this role, you will be responsible for providing support and coordination for Pediatric clients who are eligible for the GAPP (Georgia Pediatric Program).
Key Responsibilities:
- Identify Pediatric clients eligible for the GAPP program and assist them in accessing program benefits.
- Coordinate with clients, parents, and physicians to ensure all required documents for state enrollment are completed accurately and in a timely manner.
- Act as the primary point of contact for clients and families, addressing any questions or concerns regarding the GAPP program.
- Ensure that all required documentation, including state documents and program renewals, are completed and submitted according to state guidelines and deadlines.
- Maintain accurate records and documentation of client interactions and program enrollment status.
- Collaborate with internal teams to facilitate seamless communication and coordination of care for GAPP clients.
- Stay up-to-date on Medicaid state-funded programs and regulations, as well as home care services, to effectively support GAPP clients.
Minimal Qualifications:
- High School Diploma or equivalent
- Strong organizational skills with the ability to manage multiple tasks and deadlines effectively.
- Excellent communication and interpersonal skills, with the ability to interact professionally with clients, families, and healthcare providers.
- Detail-oriented approach to documentation and record-keeping.
- Ability to work independently as well as part of a team, demonstrating flexibility and adaptability in a fast-paced environment.
Preferred Qualifications:
- Bachelor's degree in healthcare administration, social work, or a related field.
- Previous experience working with Medicaid state-funded programs and/or home care services.
- Knowledge of Georgia Pediatric Program (GAPP) guidelines and procedures
- Proficiency in understanding the unique needs and challenges faced by children with physical, functional, and mental disabilities.
